Synovial plicae of the knee
نویسنده
چکیده
Synovial plicae represent inward folds of the synovial lining of the knee joint capsule, which are considered vestiges of a membranous knee joint partition present during foetal development. Three such folds are found with regularity in the human knee, but most are asymptomatic and of little clinical consequence. Plicae are more commonly seen in young adults, and can be an important cause of anterior knee pain, presenting with a plethora of symptoms. Most patients give a history of blunt trauma or an increase in repetitive sporting activities, but any form of chronic or transient synovitis can cause the plica to lose its elasticity by becoming inflamed and thickened. Such changes can affect suprapatellar and medial para-patellar plicae, which may bowstring and impinge in the patello-femoral joint during flexion, subsequently leading to localised chondromalacia. As the clinical picture is not well defined, a high degree of suspicion is required. MRI might be of value in the diagnostic assessment. The clinical diagnosis, however, is one of exclusion, and other more common pathologies relating to antero-medial knee pain should be considered first. Arthroscopy remains the gold standard for treatment of pathological plicae, although intraplical injection and physiotherapy might be beneficial in patients with a short history of symptoms. Complete excision is recommended and provides satisfactory results especially if the plica represents the only intra-articular pathology. & 2004 Elsevier Ltd. All rights reserved.

The synovial plicae in the knee joint.
OBJECTIVE To investigate and evaluate the prevalence of 4 kinds of synovial plicae named according to patella, which are inferior, medial, lateral and superior. METHODS We evaluated plicae in 318 knee arthroscopies through video records, obtained from January 1994 to December 2002 in the Arthroscopy Unit, Department of Orthopedic Surgery, Faculty of Medicine, Trakya University, Edirne, Turkey...
متن کاملSynovial plicae of the knee joint: the role of advanced MRI.
Synovial plicae are normal anatomical structures of the knee that may become symptomatic. MRI is an established technique for evaluating the anatomy of the knee, and it is a valuable tool for detecting plicae because of its high resolution resulting in increased tissue characterisation. At MRI, knee plicae appear as low-signal-intensity structures of variable size and thickness, and they are be...
متن کاملTreatment of Pathological Synovial Plicae of the Knee
OBJECTIVE To analyze the incidence, clinical significance, and clinical manifestations of pathological synovial plicae of the knee. MATERIAL AND METHODS Between 2002 and 2006, 63 patients with pathological synovial plicae of the knee were studied. Of those 63 patients, 21 had the diagnosis confirmed by previously performed magnetic resonance imaging. All of the patients initially underwent co...
متن کاملLocalized Nodular Tenosynovitis Originated near the Medial Plicae
The authors report a case of localized nodular tenosynovitis originating from the medial plicae of the knee that was grossly observable, palpable, and symptomatic. A 41-year-old male visited our hospital with a complaint of knee pain that began 4 years ago. He complained of intermittent catching and a mass that appeared in the medial side of the knee 18 months ago. On magnetic resonance imaging...
متن کاملPlica: Pathologic or Not?
A fold that occurs within a joint is referred to as a plica synovialis. Three such plicae are seen with regularity within the human knee joint. These folds are normal structures that represent remnants of mesenchymal tissue and/or septa formed during embryonic development of the knee joint, and can be seen during arthroscopic inspection of the knee joint. Controversy exists within the orthopaed...
